Waiver of Liability Agreement

Notice to all persons submitting registrations on the SOURCEDANCE On-Line Registration System:

By entering and submitting registrations you are agreeing to the following:

AGREEMENT:
I confirm that I am 18 years of age or older and am the individual responsible for payment of any registrations submitted hereinafter.

I hereby re-confirm that by having entered the Online Registration System, I Acknowledge that I have read and understand the SOURCEDANCE On-Line Registration & Payment Options and I further confirm that I agree to the terms & policies set forth therein and I want to submit registrations on-line.

As an adult (18 years of age or older) and/or parent and/or legal guardian of the student(s)/ convention participant(s)/ workshop participant(s)/ Hollywood Competition competitor(s)-participant(s)/ Hollywood Tour participant(s) I am about to register or have registered in previous registrations, I am aware that all forms of dance is a strenuous activity that involves a high level of activity and energy which may place the student/ convention/ workshop/ Hollywood Competition/ Hollywood Tour participant(s) at risk of pulled muscles, sprained ankles and/or other accidental injuries. I, as an adult understand and acknowledge that student/ convention/ workshop/ Hollywood Competition/ Hollywood Tour participant(s) is/are voluntarily participating in these activities with knowledge of the risks involved and the parent/ legal guardian/ student/ convention/ workshop/ Hollywood Competition/ Hollywood Tour participant/ attendee(s) assume any and all risks of injury. If any injury does occur, I hereby agree to indemnify, defend, and hold harmless SOURCEDANCE, its officers, agents, employees, representatives, volunteers and sponsors from and against any and all claims demands, and defense costs, actions, liability or consequential damages of any kind or nature arising out of or in connection with the SOURCEDANCE event or failure to perform, except those which arise out of the sole negligence of SOURCEDANCE. I understand that SOURCEDANCE , the convention/ workshop/ Hollywood Competition/ Hollywood Tour event site/venue and the instructors are not liable for personal injuries, or loss of or damage to personal property.

In addition, I authorize SOURCEDANCE the use of the image of the registered dancer(s), registered observers, registered chaperones(s) and/or my image for any and all advertising purposes. I further certify the information provided is true and correct. As an adult and/or parent and/or legal guardian of the student/ convention/ workshop/ Hollywood Compettion/ Hollywood Tour participant(s), I am in the process of registering, I acknowledge, agree and hereby authorize the adding of the participant(s) and/or my email address to the SOURCEDANCE News & Updates mailing list. I further acknowledge that I will have the ability to subscribe to other event location mailing lists or 'unsubscribe' from the mailing list, should I decide to do so at a later date.

I further acknowledge, understand and agree to SOURCEDANCE's policy that, if a faculty member is not available, a suitable replacement will be made.

Emergency Treatment Authorization for All Dancers

As an adult and /or parent and/or legal guardian of the student(s)/ convention/ workshop/ Hollywood Competition/ Hollywood Tour participant(s) I have registered or am preparing to register, I authorize any licensed physician to render necessary emergency treatment for injury or serious illness and I will assume all financial responsibility for such treatment. I authorize SOURCEDANCE to choose a doctor in case of emergency.
